Advancing Health Equity - Development of Person-Centered Interventions to Improve Adherence to Cardiac Rehabilitation (HeartEQ)

The project is a PhD project aiming at developing and testing an organizational model for acute cardiac nursing designed to reduce health inequalities by strengthening organizational health literacy and continuity of care. The model will be called Heart-Equity-Model.

Through three interrelated sub studies, the PhD project investigates how experiences of patients with multiple chronic conditions and their relatives during acute admission due to cardiovascular disease influence adherence to follow-up care and rehabilitiation, and how person-centered nursing practice can support a coherent and meaningful patient trajectory that facilitates adherence til follow-up care and rehabilitation.
