Abstract

Background and aim:
Approx. 300,000 Danish individuals are living with Chronic Obstructive Pulmonary Disease (COPD). Exacerbations are frequent and associated with decreased quality of life and elevated risk of hospitalization. Notably, 16 % of hospitalized COPD patients die within 30 days from the day of admission.
Use of telemedicine and remote monitoring of vital signs enables clinicians to track patients’ physical signs and respiratory symptoms using a wide range of technical equipment for the early detection of exacerbations. In Region Zealand, several telemedicine models have been testedand , especially PreCare has shown high levels of satisfaction and sense of security among COPD patients. As PreCareKOL is a is a new offer, there is currently a need for knowledge about how to provide care and treatment through a telehealth solution to patients with COPD. The first step in improving care is to know the characteristics of the population and its care needs.
AIM: To describe the characteristics of the PreCareKOL population in Region Zealand and explore their care needs through analyzing content from the documented electronic health record (EHR) notes.
Methods and material:
Study 1 will describe the characteristics of the PreCareKOL population in Region Zealand based on longitudinal cohort data.
Study 2 will explore the care and support needs of the PreCareKOL population by qualitatively analyzing the content from the documented electronic health record (EHR).

Results:
The results will be avaible in the beginning of 2024. However, preliminary results show a trend towards a decreasing number of acute admissions after being enrolled in the project PreCareKOL
Discussion and Conclusion:
The anticipated results are expected to contribute to the development of a nursing intervention. Addidtionally, the results will be used in the development of new telemedicine solutions in Region Zealand, as well as being disseminated d nationally to address national strategies for telemedicine intervention.
